Travel Outside the U.S.

To begin your service request, complete the International Student Request Form. PDF [239 kb] Attach the following documents to this form and submit them to the ISS office.

1. Attach your original Form I-20, pages 1 and 3 to the International Student Request Form.
2. Bring documents to the ISS Office.
