Replace Form 1-20

To begin your service request, complete the International Student Request Form. PDF [239 kb] Attach the following documents to this form and submit them to the ISS office.

1. Completed and signed Declaration & Certification of Finances (DCF) form.
2. Photocopy of Form 1-20 pages 1 and 3, if available.
3. Bank statement(s) (yours or a sponsor's); see DCF for instructions. Must reflect a three-month account history.
4. If using a sponsor's bank statement, sponsor's signed and dated letter of support.
5. If your Form 1-20 was lost, include a letter of explanation as to when and how the Form 1-20 was lost. Attach a copy of the police report, if available.
6. If you are applying for an extension of your Form 1-20, you need to submit a letter from your program advisor/academic advisor explaining the reason for the extension and the new program end date.
