Form I-20 for dependents

To begin your service request, complete the International Student Request Form. PDF [239 kb] Attach the following documents to this form and submit them to the ISS office.

1. Original Form 1-20 pages 1 and 3.
2. Proof of relationship to student for each person listed on the Information for Dependents and Visitors chart located on the ISS Request Form.
3. Declaration & Certification of Finances (DCF) form. bank statements, and sponsor's letter of support verifying that sufficient funds are available to pay all dependents' expenses while in the U.S.
